Course Approvals

Massage Therapist 

Massage Therapy is governed by the Arizona State Board of Massage Therapy.

To obtain a Massage Therapist License, the State of Arizona* requires the following at minimum:

Regular License

1. Successful completion of min. 700-hour Massage Therapy program.
2. Submit a FD-258 fingerprint card and pass state and national criminal history record check.
3. Proof of Arizona Residency.
4. Proof of citizenship or lawful presence 
5. Be of good moral character
6. If the applicant graduated from a school outside of Arizona, they must also have successfully completed a national licensing exam (MBLEx or NCBTMB) OR

 License by Reciprocity

1. Hold a license from another state or United States territory for at least two of the last five years immediately preceding the application; or,
2. Hold a current certification from the National Certification Board for Therapeutic Massage & Bodywork (NCBTMB) and have graduated from a Massage Therapy school with a minimum of 500 hours; or,
3. Hold a license from another state or United States territory for one year, be active at the time of application, and have established residency in the State of Arizona.

The Massage Therapist License is subject to renewal every other year on the licensee’s birthday and expires unless renewed.

Physical Therapist 

Physical Therapy and Physical Therapy Assistants are governed by Arizona State Board of Physical Therapy

To obtain a Physical Therapist License, the State of Arizona requires the following at minimum:

• Graduate from a CAPTE-accredited Physical Therapy program
• Pass the National Physical Therapy Exam (NPTE)
• Pass the Arizona Jurisprudence Exam
• Provide a valid Arizona Fingerprint Clearance Card for a background check

Physical Therapist Assistants 

To obtain a Physical Therapist Assistant License, the State of Arizona requires the following at minimum:

• Graduate from a CAPTE-accredited Physical Therapy Assistant program
• Pass the NPTE for PTAs
• Pass the Arizona Jurisprudence Exam
• Provide a valid Arizona Fingerprint Clearance Card for a background check

*The PT and PTA license is subject to renewal every even year by August 31st.

Athletic Trainers 

Athletic Trainers are governed by Arizona Board of Athletic Training. To obtain an Athletic Trainers License, the State of Arizona requires the following at minimum:

• Graduate from an accredited Athletic Training program or hold a bachelors degree or higher
• Pass the National Exam (BOC) 
• Pass the Statutes and Rules Exam
• Provide a valid fingerprint clearance card
• Provide a verification of licensure form from all states that a license has been held
• Complete the statement of citizenship form and provide proof of citizenship
• Provide proof of your current, unexpired CPR Card

*The Athletic Trainers license is subject to renewal every odd year by December 31st.
